I got up to go to bathroom and there was blood in my underwear, and then when I wiped there was some more.
I am 38 weeks pregnant. What does this mean?

My guess would be this means you're going to be meeting your baby soon. If there was mucous mixed in with it, it's most likely a bloody show, which indicates labor will most likely be starting soon.
The cervix seals with a mucous plug during pregnancy. As it begins to dilate for your baby it's not uncommon for blood vessels to rupture and then you get blood mixed in with it.
If it was just blood it could indicate issues with your baby's placenta. Or it could be another labor indication - bright red blood can be common in labor.
In any event, you should call your doctor or midwife and describe the blood to them to see what they think, that's the best way to know that everything is ok.
